Instructions For Submitting Your Story

1. Print the Personal Information Form (PDF format*) and fill it out in its entirety, so that we will know how to contact you.
   
   
2. Also, print and sign the Release Form (PDF format*) giving Alzheimer’s Disease Research permission to use your story and any photographs that you submit.
   
   
3. Write your story in 2,000 words or less (that’s no more than 3 single-spaced pages). One page is usually sufficient. Include information about your (or your loved one’s) diagnosis, the course of the disease, what day-to-day life is like living with the condition, and what you can expect from the future. Tell us how your life has changed since the onset of the condition.
   
   
4. Include 3 – 4 color photographs of yourself or your loved one that we have your permission to publish.
   
   
5. Mail the completed forms and photographs to:
