Memorial Tribute Page Sample
This is an example of what a memorial tribute page looks like once it's created.
Memorial funds typically pay tribute to a special loved one who is no longer living. The fund champion will be asked to supply the full name (along with title, prefix, and suffixes) of the person being honored and then the name and contact information for a Next of Kin representative who can receive email notifications when gifts are made to the fund.

Linda was a wonderful mother, grandmother, wife and friend. She loved spending time with her husband and family, gardening, playing softball, and reading poetry. She had a contagious laugh and always had a kind word to say to those around her. Even after Alzheimer’s disease had ravaged her mind, she still found a way to smile. Her smile was always a beacon of light to those around her.
Growing up, I remember when she used to push me and my sisters on our little red tree-swing. How I used to love that! On snowy days, she would tell us to go outside and play. Of course, once we returned home from the cold, she would have hot chocolate, tomato soup, and turkey sandwiches (cut into perfect little triangles) already waiting. I can recall the cool fall nights where she would help us decide what we wanted to dress up as on Halloween. One time, she even sewed one of my costumes by hand. I was a dragon that year and one a prize. My sister dressed up as the Sun Maid raisin lady!
I remember all of the wonderful things that she did as I grew older too. She was always there for me, and for all of us, regardless of what directions we took or what happened in our lives. I especially loved to watch her eyes light up whenever she saw one of her grandchildren. After Alzheimer’s took hold, that familiar light began to fade and with it her life memories faded as well. Even though beautiful memories like these were lost to her in the end, they will remain with me through my journal entries, the stories that I’ve told my children and memorials like these.
Linda’s struggle with Alzheimer’s disease has come to an end and she is in a better place now. I know that she would not want us to be sad for her. She would want us to think of her in better times and support research in order to find a cure for this disease once and for all.
