Instructions For Submitting Your Story
- Print the Personal Information Form (PDF, 1 page) and fill it out in its entirety, so that we will know how to contact you.
- Also, print and sign the Release Form (PDF, 1 page) giving Macular Degeneration Research permission to use your story and any photographs that you submit.
- Write your story in 2,000 words or less (that’s no more than 3 single-spaced pages). One page is usually sufficient. Include information about your (or your loved one’s) diagnosis, the course of the disease, what day-to-day life is like living with the condition, and what you can expect from the future. Tell us how your life has changed since the onset of the condition.
- Include 3 – 4 color photographs of yourself or your loved one that we have your permission to publish.
- Mail the completed forms and photographs to:
