Tell Us Your Story

Do you or a loved one suffer from macular degeneration? We are looking for the true-life stories of real people who need this research to go forward.

Your story, along with the photos you provide, could be published in a booklet or newsletter that will be shared with the general public. This booklet will be provided as a nonprofit educational effort for anyone who is interested in learning more about why macular degeneration research is desperately needed. Its release will be announced to journalists nationwide to help bring public attention to this vital issue, and to the human faces behind all the talk. We may also ask if you would like to participate in one of our public service videos, which will be released to television stations nationwide. Participation is strictly voluntary, and no financial compensation will be given to participants.

Your story is important. Don’t let it go untold!

Instructions For Submitting Your Story

  1. Print the Personal Information Form (PDF, 1 page) and fill it out in its entirety, so that we will know how to contact you.
  2. Also, print and sign the Release Form (PDF, 1 page) giving Macular Degeneration Research permission to use your story and any photographs that you submit.
  3. Write your story in 2,000 words or less (that’s no more than 3 single-spaced pages). One page is usually sufficient. Include information about your (or your loved one’s) diagnosis, the course of the disease, what day-to-day life is like living with the condition, and what you can expect from the future. Tell us how your life has changed since the onset of the condition.
  4. Include 3 – 4 color photographs of yourself or your loved one that we have your permission to publish.
  5. Mail the completed forms and photographs to:
