I. Notification of Donor Intent

Whatever means you use to deliver a gift of stock, either the donor or the transferring broker must inform our organization of the following information for your financial audit and acknowledgment purposes. Without all of the listed information, the stock transfer cannot be processed. A stock gift instruction form is provided for your convenience. You will need to provide the following information:
Donor's Name
Donor's Complete Address
Name of the Securities being transferred.
Number of Shares being transferred
Specific American Health Assistance Foundation program to which the gift should
be directed (Alzheimer's Disease Research,
National Glaucoma Research, or Macular Degeneration
Research)
This information should be provided by the donor or by the broker
